Abstract: | 為探討台灣地區的外傷型態與國外的異同,特以外傷登錄為工具,研究現今台灣地區傷害型態之分佈及提供防治之方法。本研究以台灣地區六所醫院急診病患中,因外傷且需住院或接受手術的病人為資料庫收集的對象,資料的收集時間自90年2月1日至90年11月30日共10個月,計有1,353人。結果顯示,以性別來看,男性為女性的2倍;年齡層共分為四層,分別為0-14歲、15-29歲、30-64歲及65歲以上等四層,其中以30-64歲所佔的比例為最高(48%)。受傷原因以交通事故及跌落所佔的比例最高,分別佔52%及29%;受傷部位以四肢最多,佔44%。受傷地點以街道馬路所佔的比例最高(52%)。車禍中以機車車禍所佔的比例最高,其中又以15-29歲及30-64歲所佔的比例明顯較高,均佔約45%。台灣目前較具公信力的資料庫為行政院衛生署每年所做的死因資料分析,但並沒有一個探討傷害的資料庫,經由外傷登錄系統,我們可以瞭解到整個台灣的傷害型態,幾乎所有重要的外傷醫療決策,如:外傷預防、到院前緊急救護系統、外傷照護品質等都需以外傷登錄為依據。因此建立一個合理實用、可靠並具公信力的外傷登錄資料庫是很重要的,也是我們未來應共同努力的目標。 In an attempt to study the feasibility of trauma registry in Taiwan, 1,353 trauma patients admitted to or operated in six different hospitals were collected over a period of 10 months from Feb. 1, 2001 to Nov. 30, 2001. The results from this data set revealed a male predominance of 2:1 in sexual distribution. They were all divided into four different age group, namely, 0-14-year-olds, 15-29-year-olds, 30-64-year-olds, and greater than 65 years old. Most injury occurred in age group of 30-64 year-olds. Motor vehicle injury and falls were the two main causes of trauma, accounting up to 52% and 29% respectively. Extremities (44%) were the most frequently injured anatomic parts. As for place of occurrence nearly 56% of trauma occurred in the street. In motor vehicle injury, motorcyclist was most frequently injured, especially in the age group of 15-29 (40%) and 30-64 (45%). The most frequent trauma in fall was slipped and fall (62%), with patient greater than 65 years old accounting up to 89% in this category. Falls from a height was most frequently seen in the age group of 15-29 year-olds. The annually published cause of death analysis by the Department of Health Taiwan is a valuable database, but it lacks details of trauma breakdown. It is only through trauma registering system could we understand more of the injury pattern in Taiwan whereby important medical policies, such as trauma prevention, emergency medical system, and trauma care could be planned. Therefore it is imminent for us to set up a feasible, practical, and reliable trauma registering database, and this will be our immediate goal. |
