    摘要: 過去動物和人類的研究中都發現腸道菌相的組成與高血壓具有一定之關聯性,但多數研究對可能影響腸道菌相的受試者飲食內容無詳細的評估,且這些研究的地區多為歐美地區,較缺乏飲食型態有顯著不同的亞洲地區資料。因此,本研究以建立台灣本土資料為出發點,使用橫斷面的研究方式探討高血壓患者的腸道菌相組成,於國泰綜合醫院老人健康檢查時招募,年齡介於65~80歲之高血壓患者20名與年齡性別相配對之健康受試者21名。收集受試者資料包括:身高、體重、身體質量指數、血壓、脈搏、血液常規與生化檢查等,並利用16S rRNA gene sequencing方法分析糞便腸道菌組成。另外,由營養師收集24小時飲食紀錄與食物頻率問卷,計算營養素攝取量以及分析飲食型態。研究結果顯示,高血壓患者的身體質量指數、腰圍與收縮壓皆顯著比健康對照組較高。飲食攝取狀況,兩組之間24小時飲食回憶的總熱量與三大營養素攝取比例皆無顯著差異,而飲食頻率問卷分析亦無顯著差異。腸道菌相分析發現,與健康對照者相比,高血壓患者的腸道菌相α與β多樣性皆無顯著差異,而高血壓患者糞便中的Bacteroides caccae以及Barnesiella屬顯著較少,Enterobacteriaceae科、Enterobacter屬、Acidaminococcus屬與Bacteroides plebeius則顯著較多。本研究結果可作為後續台灣高齡高血壓患者之腸道菌相特徵的重要參考。
    Previous studies have identified a correlation between gut microbiota composition and hypertension in both humans and animals. However, these studies often lack detailed dietary assessments of participants and are predominantly conducted in Western populations, with limited data from Asian regions with distinct dietary habits. This cross-sectional study aimed to characterize the gut microbiota of hypertensive elderly patients in Taiwan. Twenty hypertensive patients and 21 matched healthy controls, aged 65-80, were recruited from Cathay General Hospital. Data collected included anthropometrics, blood pressure, blood tests, and dietary intake via 24-hour recalls and food frequency questionnaires. Using 16S rRNA gene sequencing, fecal microbiota composition was analyzed. Results indicated hypertensive patients had higher BMI, waist circumference, and systolic blood pressure. Dietary intake showed no significant differences in total calories or macronutrients. Gut microbiota analysis revealed hypertensive patients had significantly lower Bacteroides caccae and Barnesiella but higher Enterobacteriaceae, Enterobacter, Acidaminococcus, and Bacteroides plebeius. This study provides important reference data on the gut microbiota characteristics of hypertensive elderly in Taiwan.
