    題名: 開發具臨床支臺齒顏色的牙科陶瓷燒結膏
    Development of dental ceramic firing pastes with clinical abutment tooth shade

    摘要: 數位牙科製程中,由於口腔掃描機的發明和普及,口掃檔案取代了實體的印模與牙齒石膏模。因為沒有石膏模型,牙體技術師需要將燒結膏擠入牙冠內冠,再插上金屬支撐柱以穩固牙冠。藉此確保陶瓷牙冠在上釉及染色的過程中不會掉落。本研究分為兩個部份,第一部分為無色燒結膏,探討5種市售燒結膏品牌及自製燒結膏的樣貌與特性。第一部分旨在開發一款台灣國產的燒結膏,並達成和市售燒結膏功能相近但價格更低的目標;第二部分為開發有色燒結膏。將自製無色燒結膏中加入不同比例的紅、黃、藍色食用色素,調配出具有臨床病患口內支臺齒顏色的燒結膏。協助牙體技術師在染色的過程可以因選擇和個案口內支臺齒相同顏色的燒結膏,而解決厚度較薄的?復體會透光、透色,產生牙冠在口內外具有顏色差異的問題,並且達到支撐牙冠的效果。
    本研究使用的主要材料為耐高溫棉及甘油。使用光學顯微鏡(OM)、掃描式電子顯微鏡(SEM)、能量散射光譜(EDS)、同步式熱重熱示差掃描分析儀(STA)和吸光度值(OD)對市售及自製燒結膏進行分析。測量燒結膏燒結後的纖維管徑與重量損失,以及在e.max CAD、艾爾創ST及MarkII陶瓷上的沾黏程度。針對有色燒結膏進行光學分析。結果顯示,本研究所開發的燒結膏具有相較其他品牌更為良好的生物相容性,並且與VITA firing paste和Sirona CEREC SpeedPaste等品牌的外貌及性質相似,可以作為陶瓷牙冠燒結及染色時的有效支撐材料。本研究的燒結膏使用後不易沾黏在內冠上、無毒,並僅需要低成本。此外,本實驗成功開發出6款具支臺齒顏色的燒結膏。牙體技術師可以挑選接近病患口內支臺齒顏色的燒結膏,並擠在內冠裡進行染色,可以有效減少口外染色與口內裝上牙冠時的顏色差異。
    In the process of digital dentistry, the widespread adoption of intraoral scanners has replaced physical impression and dental stone models with digital scans. Since the absence of stone models, dental technicians have to inject firing paste into the inner crown and then insert a metal pin in order to stabilize the crown. Ensuring it does not fall off when putting the crown into the furnace as well as during the strengthening, glazing and staining process. This study consists of two parts. The first part focuses on original firing pastes. Investigating the appearance and characteristics of five different brands and our self-made firing paste. The aim of the first part is to develop a firing paste that is made in Taiwan while the functions similar to commercial ones but at a lower cost. The second part concerns firing pastes with shades, where different proportions of red, yellow, and blue food colorants are added to our self-made firing pastes to create shades matching the colors of abutment teeth in patients' mouths. This enables dental technicians to solve the problem of discolored crowns resulting from thin and translucent restorations by selecting firing pastes with colors same as the clinical abutment teeth when staining. Dental technicians can achieve the desired color both intraorally and extra-orally. Simultaneously, this can also earn adequate support for dental crowns during the staining process.
    The main material used in this study is fire-proof cotton and glycerol. Five different brands of firing paste and self-developed firing pastes were evaluated using Optical Microscope (OM), Scanning Electron Microscope (SEM), Energy Dispersive X-ray Spectroscope (EDS), Simultaneous Thermogravimetric Analyzer (STA) and optical density (OD value). Measurements included the fiber diameter, weight loss, and adhesion to three types of ceramics (e.max CAD、Upcera ST and MarkII) were made after the firing paste were sintered in the furnace. Optical analysis was carried out on firing pastes with shades. The results indicate that the firing paste developed in this study exhibits better biocompatibility compared to other brands. It is similar in appearance and properties to brands such as VITA firing paste and Sirona CEREC SpeedPaste, and can serve as an effective support material for sintering and staining ceramic dental crowns. Additionally, it is less likely to adhere to the inner crown, non-toxic, and requires only low cost. This study successfully developed six firing pastes with colors matching abutment teeth. By selecting shades closely resembling the colors of abutment teeth in patients' mouths and applying them into the inner crown before staining, it effectively reduces color discrepancies between lab-side staining and intraoral crown placement.
