摘要:	過去文獻報告顯示老年人髖骨骨折後一年內死亡率明顯增加，因此醫界一直呼籲老年人要注意「保密防跌」，即保持骨骼密度、預防跌倒。隨著醫療技術與照護品質的進步，以及民眾普遍開始注重維持骨骼密度，我們嘗試探討2016年至2021年因為髖骨骨折接受手術的老年人，一年內死亡率是否有下降，並探討老年人人工髖關節置換術後一年內死亡主要是哪些原因造成。本研究使用TMUCRD進行觀察性研究，針對50歲以上的老年人經過髖骨手術後一年內的死亡率做探討。本論文初步證實，在2016年至2021年之六年期間臺灣中老年人接受人工髖關節置換術後一年內死亡率與過去文獻相比，沒有顯著差異，但是透過TriNetX跨國電子病歷網路資料庫之全球網路（Global）及美國地區（US）的資料進行外部資料分析，以及與國外過去的文獻數據相比，可以初步的看到臺灣的一年內死亡率相對較低，表示臺灣整體的醫療品質有一定的水準。我們發現2016年至2021年TMUCRD入院接受髖部手術後一年內的死亡率為13.113%，低於全球（15.595%）及美國地區（14.989%）。三個資料庫的六年內入院平均年齡分別為79.5歲、71.1歲及76.0歲，女性入院及死亡的人數皆大於男性，死亡率則是男性大於女性。 Past literature indicates a significant increase in the one-year mortality rate among elderly individuals following a hip fracture. Therefore, the medical community has consistently emphasized the importance of "maintaining bone density and preventing falls." With advancements in medical technology and quality of care, as well as a growing public awareness of bone density maintenance, we aimed to investigate whether the one-year mortality rate for elderly individuals undergoing hip fracture surgery from 2016 to 2021 has decreased. We also explored the primary causes of one-year mortality after hip replacement surgery.Using data from the Taipei Medical University Clinical Research Database (TMUCRD), we conducted an observational study on the one-year mortality rate for individuals aged 50 and above who had undergone hip surgery. Preliminary findings indicate no significant difference in the one-year mortality rate from 2016 to 2021 compared to past literature. However, through external data analysis using the TriNetX international electronic medical records network, we observed that Taiwan's one-year mortality rate is relatively lower, indicating a certain level of overall medical quality in Taiwan.We found that the one-year post-surgery mortality rate for patients from 2016 to 2021 was 13.113%, lower than the global rate of 15.595% and the American rate of 14.989%. The average ages were 79.5 years in Taiwan, 71.1 years globally, and 76.0 years in the US. More women than men were admitted and died, though the mortality rate was higher in men.
