    摘要: 區塊鏈(Blockchain)自2009年發表至今,已被認為是極具潛力之技術。區塊鏈除了在被開發初期,常用於加密電子貨幣(Cryptocurrency)之流通平台外,近期也被運用於醫療照護之領域,區塊鏈之三大特性:不可竄改性(Immutability)、去中心化(Decentralization)、隱私性(Privacy)已被提出可強化使用電子健康紀錄(EHR)的安全性。同時,非同質化硬幣(Non-fungible Token,NFT)則是基於區塊鏈平台上一種具有唯一性且不可替換的虛擬單元,通常可被利用於獨一無二的電子化品項的交易,而這正是作為具有安全性的醫療資料庫管理所需之要項之一,因此本論文使用NFT非同質化代幣之區塊鏈的技術運用於EHR電子健康紀錄,將使用乙太坊區塊鏈及IPFS星際資料庫混合的EHR資料庫形式,分別將EHR擁有者上傳檔案較大之原始檔案儲存至IPFS,另外則將檔案較小的其他EHR文本資料上傳至區塊鏈上作為NFT,供有需要之EHR使用者在察看簡單內容後,以虛擬貨幣換取原始EHR資料。本論文提出之混合式資料庫目的將測試混合式資料庫之EHR管理架構及作為NFT使用之可行性,期望未來可用混合式區塊鏈資料庫來維持EHR資料庫的分散式特性來達到對EHR做更有效率且更安全的管理。

    在此論文之驗證結果中,使用乙太坊區塊鏈以及IPFS做為測試環境,以不同的節點模擬可被允許的EHR使用角色向區塊鏈提出需求,並透過符合ERC-721標準之智能合約的控制,讓使用者以虛擬貨幣購買NFT化之EHR資料,並且可於區塊鏈中獲取IPFS儲存位址,進而由混合式資料庫本體獲得EHR原始資料。EHR擁有者發起可進一步作為回饋之健康NFT區塊鏈架構,利用區塊鏈上的價值回饋,去促使個人或是機構上傳個人電子化健康資料,同時也可確保醫療資料在保有安全性的資料架構下被使用。
    Since the blockchain published in 2009, it has been considered as a technology with enormous potential in cryptocurrency or financial area. In healthcare industry, the three major features in immutability, decentralization, privacy also make the blockchain increase the datasecurity for using electronic health records (EHR) in a peer-to-peer architecture. Simultaneously, the non-fungible token (NFT) are a new type of unique and indivisible blockchain-based tokenswhich is usually be used for transactions of irreplaceable electronic assets. This feature is one of crucial issue for being a secure healthcare database. Therefore, the aim of this thesis is to use the blockchain technology with NFT to apply to an EHR electronic health records healthcare database. It combines the IPFS (InterPlanetary File System) architecture for large EHR raw data itself and the blockchain architecture for the thinner informations of the EHR. Only the authorized blockchain user, also as EHR consumer can full-accessed by consume with cryptocurrency token. The purpose of the hybrid database proposed in this paper is to test whether the EHR management structure of the hybrid database is feasible. In the future, the hybrid blockchain database can achieve to a more efficient and safer EHR data management by using decentralized blockchain database.

    In the verification results, the Ethereum blockchain and IPFS are used as the test environment, and different nodes are used to simulate the authorized EHR demander roles. Complianced with the ERC-721 smart contract standard, the smart contract enables the demander to purchase EHR with Cryptocurrency, and get the IPFS address then obtain the raw EHR data from the hybrid database. EHR owners initiate the hybrid health NFT blockchain architecture as a feedback, using the token feedbacks by the blockchain to encourage individuals or organizations to upload personal EHR to promote the medical industry use of health data in a secure data management scenario.
