    摘要: 腫瘤抗性是癌症治療失敗的主要原因。腫瘤抗性可分為內生性(intrinsic)與外生性(extrinsic)兩種因子。內生性抗性主要透過癌細胞的信號傳導、抑制免疫辨識和腫瘤突變進一步促進促成了癌幹細胞特性(cancer stemness)。外生性抗性主要透過癌細胞與腫瘤微環境 (TME) 的相互溝通,特別是與環境中的基質細胞交互作用,並且細胞素等因子吸引更多抑制性免疫細胞來創造利於腫瘤生長的環境。此研究題出了兩種治療模式,利用營養以及免疫途徑來克服腫瘤抗性。在營養方法中,過去已證實在數個肺癌細胞株使用酵母硒和魚油可以誘導細胞株的凋亡,同時減少對標靶藥物Gefitinib的藥物抗性。在我們的研究中,看到酵母硒和魚油的組合在具有癌幹細胞特性的A549球型細胞株上,不僅可減低該細胞株的癌幹細胞特徵,同時還能降低對鉑金類化療藥物的抗性。而這個特性的主要途徑就是靠AMPK的活化來達成。此外,在免疫學方法中,我們也在嘗試開發樹突細胞癌症疫苗,並為此設計了癌症新抗原的預測模型。透過此模型,能準確預測個別腫瘤的新抗原,可提升癌症疫苗的效果。然而腫瘤本身的抗性,特別是剪低免疫辨識與抑制性的腫瘤環境皆是癌症疫苗失敗的重要因子。因此我們希望透過酵母硒和魚油的組合來降低癌細胞抗性,激活內在 AMPK 通路,進而加強免疫辨識,提升癌症疫苗的效用。綜上所述,我們的研究提供一個透過營養以及免疫學途徑來共同對抗腫瘤抗性,輔助癌症治療的方法。
    Tumor resistance is a major reason for the failure of physiological and therapeutic interventions for cancer. Intrinsic resistance consists of changes in the signaling pathways of cancerous cells, suppression of immune antitumor response, and tumor mutations, which further contributes to cancer stemness. Extrinsic resistance entails host-mediated factors and local tumor microenvironment (TME) creating a tumorigenesis environment by interaction with stroma cells and recruiting inhibitory immune cells. To overcome tumor resistance, our study demonstrated two modes of therapeutic approaches, nutritional and immunological. In the nutritional approach, the combined selenium yeast and fish oil were administered, This combination had demonstrated its ability to elevate the induction of apoptosis in several lung cancer cell lines as well as retarding acquired resistance to Gefitinib. In our study, this combination further inhibited stemness characteristics and the resistance to Cisplatin of A549 sphere cells via the AMPK pathway. In the immunological approach, we attempted to develop a cancer vaccine for which a prediction model, a solid pipeline, for neoantigens were designed. By incorporating the nutritional combination, not only hinders the stemness of tumor cells, but also enhances antigen presentation, could significantly aid the efficacy of cancer vaccine. Taken together, our study provides insights into the possibility of nutritional and immunological approaches in surmounting tumor resistance
