    題名: Vitamin D, Nutritional Status, and Functional Parameters in Taiwan Nursing Home Older Dwellers: A Comparative Study of Well-Nourished and At-Risk Individuals

    摘要: More research is needed to study how insufficient nutrition and low vitamin D levels impact the functional abilities of elderly nursing home residents. The objective is to investigate this matter thoroughly. This study used a cross-sectional design. Participants were 152 Taiwan nursing home older dwellers aged ?60 years; 45 were categorized as well-nourished, and 107 were at risk of malnutrition defined by the Malnutrition Universal Screening Tool (MUST). Functional parameters included hand grip strength (HGS) and 6-meter walk speed assessments. Low muscle strength (low HGS) and slow walking speed was defined based on the Asian Working Group for Sarcopenia 2019 consensus. Serum 25-hydroxyvitamin D, an index of vitamin D levels, were 31.64±9.05 ng/mL and 22.95±9.29ng/mL in the well-nourished and at-risk groups. Covariates included some variables: age, gender, height, total calorie intake, body mass index, and serum albumin concentrations. The correlation between vitamin D serum levels and functional parameters was not observed. Multi-nominal logistic regression categorized participants into three vitamin D statuses. The risk of low HGS association with vitamin D serum concentrations revealed no significance as the prevalence of serum vitamin D deficiency (<20 ng/mL), insufficiency (20-29 ng/mL), and sufficiency (?30ng/mL) was 71.4%, 70%, and 77.4%. Similarly, the analysis for the risk of slow walking speed did not show an association with vitamin D serum levels: deficiency;73.5%, insufficiency; 86%, and sufficiency; 86.8%. These findings provide clinical evidence that the protective role of vitamin D on human muscle metabolism may not be, at least in the studied population.
