    題名: The contribution of Indonesian governmental policy on the change of exclusive breastfeeding inequalities: the 2002—2017 Indonesia Demographic Health Survey

    摘要: Background: Exclusive breastfeeding (EBF) rate in Indonesia outperformed the global target. However, it is unclear if the health policy or socio-demographic development contributed to EBF rate equalities among different subgroups. This study aims to evaluate the contribution of Indonesian governmental policy on the change of trend and subgroup differences in EBF.

    Methods: A repeated cross-sectional design was applied. This study used publicly-accessible data from the Indonesia Demographic and Health Survey (IDHS), which are IDHS in 2002/2003, 2007, 2012, and 2017. We selected samples of all ever-married women aged 15–49 years. A dependent variable in this study was EBF. We compared EBF differences among subgroups based on each of five sociodemographic variables and two non-sociodemographic variables. A desk review was conducted to evaluate the contribution of Indonesian governmental policy.

    Results: Overall, the EBF rate was 50.69% (95% confidence interval [CI] 44.13% to 57.24%) in 2017, which was started to increase between IDHS 2007 and 2012. The subgroup differences also decreased in wealth index, occupational status, place of delivery, and birth attendant. However, we still identified the regional differences from 2002/2003 to 2017, e.g., the EBF rate of Kalimantan was 24.35% (95% CI 36.75% to 11.94%) lower than that of Java-Bali in 2017. Of seven governmental policies, we considered the baby-friendly hospital policy launched in 2008 potentially associated with the reduced inequalities in place of delivery and birth attendant.

    Conclusions: Indonesian governmental policies regarding EBF partially contributed on the change of EBF inequalities. However, future studies need to identify policy improvement to close the gaps.
