摘要: | 本研究旨在瞭解社區志工對於失智症知識、對失智症者態度程度,以及提供失智友善行為的意願,以臺北市12行政區健康服務中心社區志工為研究對象,為橫斷性研究,以分層等比方式抽樣,採用研究對象自填問卷收集資料,有效問卷共404份,有效問卷率99.5%。研究結果摘述如下: 一、研究對象以女性為多數,平均年齡為64.61歲,教育程度以高中(職)占多數,除了擔任志工服務以外沒有任職其他工作及志工年資10年以下為最多;近半數有接受過失智症相關教育訓練、65.1%沒有照顧過失智症者經驗。 二、失智症知識平均答對率為42.9%, 4個構面中以「照護注意事項」平均答對率54.5%最高,「溝通與行為」平均答對率28.8%最低;各題平均得分為0.29分至1.70分,以「運動對失智症患者通常是有益的」答對率最高(75.7%),「突然發生認知問題是常見失智症類型的特徵」答對率最低(10.1%)。 三、對失智症者的態度各題平均得分為1.34分至2.37分,九成以上的研究對象覺得當家人有罹患失智症時不會感覺羞恥(94.6%),並且認為失智症者應該多多參與社區活動(96.5%);有59.9%認為失智症者能與他人建立良好關係,但是又覺得難與失智症者溝通(62.1%),甚至覺得失智症者會造成他人困擾(62.2%)。 四、提供失智友善行為意願各題平均得分為1.88分至2.37分,九成以上研究對象若遇到疑似失智症者時,能不勉強的將失智症者挽留在原地,陪同等候支援(91.6%),主動辨別其需要他人協助(94.8%),主動關心需求(95.3%),並且聯絡當地派出所或撥打110請求協助(97.5%);七成的研究對象有主動加入失智友善天使之意願(70.8%)。 五、研究對象的年齡、失智症知識、對失智症者的態度與提供失智友善行為意願達顯著正相關,其中失智症知識程度與對失智症者的態度之間也達顯著正相關,且年齡愈大愈有提供失智友善行為之意願,而失智症知識程度愈佳,對失智症者的態度會愈好,更能提供失智友善行為意願。 六、所有變項可解釋提供失智友善行為意願總變異量的37%。 依據研究結果建議將認識失智症、了解「失智友善天使」及與失智症者溝通技巧,納入社區志工與公共衛生護理人員培訓課程內,辦理研討會或論壇讓社區志工分享服務失智症者的經驗,提升社區民眾主動提供失智友善行動的意願。 This study investigated the knowledge of dementia, attitudes toward people with dementia, and willingness to dementia-friendly behaviors among community volunteers. It uses the community volunteers from health centers in 12 districts of Taipei City as the research object, to carry out the cross-sectional study. A stratified and equal-ratio method is used for sampling, and the subjects are asked to fill out questionnaires to collect data. There are 404 valid questionnaires in total with an effective questionnaire rate of 99.5%. The study outcome is summarized as below: 1.Most of the subjects are female, and the average age of the subjects is 64.61 years old. Graduates of senior (vocational) high school accounts the most in the level of education. Most of them are either only working as volunteers without other full-time jobs or with a seniority of voluntary experience for less than ten years. Nearly half of them received education training related to dementia and 65.1% of them are with no experience of taking care of patients with dementia. 2.The average correct rate of dementia knowledge is 42.9%. Among the four dimensions, “Care considerations” is with the highest average correct rate in 54.5% while “Communication & behavior” is the lowest in 28.8%. The average scores for each question is between 0.29 and 1.70. The question of “Exercise is generally beneficial for people experiencing dementia” has the highest correct rate (75.7%) while “The sudden onset of cognitive problems is characteristic of common forms of dementia” has the lowest correct rate (10.1%). 3.The questions for attitudes towards patients with dementia show the average score for each question between 1.34 and 2.37. More than 90% of subjects “If my family member came to have dementia, I would be ashamed” (94.6%). and believe patients with dementia shall participate in community activities more (96.5%). There are 59.9% of them believing patients with dementia can establish good relationship with others while they also feel it is difficult to communicate with patients with dementia (62.1%) and even thinking patients with dementia will cause troubles to others (62.2%). 4.The questions for aspiration of providing dementia friendly behavior reveal an average score for each question between 1.88 and 2.37. More than 90% subjects are able to identify the patients with dementia who need help if they encounter any suspected patients (94.8%), actively check what they need (95.3%), not forcibly keep the patients with dementia at the original place and accompany them to wait for support (91.6%) and contact the local police station or call 110 to ask for help (97.5%). Around 70% of subjects are willing to participate in the activity of dementia-friendly angel voluntarily (70.8%). 5.There is a significant positive correlation between the age of subjects, the knowledge of dementia, the attitude towards patients with dementia and aspiration of providing dementia friendly behavior. Among them, it also shows significant positive correlation between the knowledge of dementia and the attitude towards patients with dementia. Besides, the subjects who are older have greater aspiration of providing dementia friendly behavior while the subjects with better knowledge of dementia carry better attitudes toward patients with dementia and are more willing to provide dementia friendly behavior. 6.All the variables can explain 37% of total variation for aspiration of providing dementia friendly behavior. According to the outcome of the study, it is suggested to include knowing dementia, understanding “dementia-friendly angel”, and the skills of communicating with patients of dementia into the training courses for community volunteers and public health nursing personnel as well as hold seminars or forums to invite community volunteers to share their experience in providing services to patients with dementia in order to enhance the aspiration among community residents to actively providing dementia-friendly behavior. |
