    題名: 運動和營養補充介入對社區老人衰弱之改善成效

    摘要: 背景:
    台灣地區推估於2026年將邁入超高齡社會,老化的過程出現衰弱因素。衰弱同時也增加憂鬱風險,造成生活品質下降;因此,如何確立合適管理策略,以有助改善老年人衰弱症狀、憂鬱症狀和生活品質值得進一步探討。
    研究目的:
    本研究探討運動和營養介入對社區老人衰弱症狀、憂鬱症狀和生活品質之改善成效。
    研究方法:
    本研究採類實驗研究設計(Quasi-experimental study design),將社區照顧關懷據點分為四組:控制組,單獨運動介入組,單獨支鏈氨基酸介入組,運動合併支鏈氨基酸補充介入組合,進行12週介入,以探討對社區老人衰弱症狀、憂鬱症狀和生活品質改善成效。統計方法包括描述統計、卡方檢定、變異數分析及廣義估計方程式(GEE)進行成效分析。
    結果:
    結果顯示衰弱分數部分,運動合併支鏈氨基酸補充組、運動組,及支鏈氨基酸補充組在衰弱症狀改善皆顯著高控制組;成對比較結果顯示,運動合併支鏈氨基酸補充組及單獨運動組改善衰弱成效顯著高於支鏈氨基酸補充組及控制組(p <0.001)。憂鬱評估部分,三組介入模式對憂鬱症狀改善皆無顯著差異。在生活品質方面,僅運動介入組顯著改善生活品質之生理健康範疇。
    結論:
    本研究12週運動合併支鏈氨基酸補充、單獨運動、單獨支鏈氨基酸補充都能夠有效降低衰弱的風險;運動合併支鏈氨基酸補充、單獨運動組、單獨支鏈氨基酸補充對憂鬱症狀改善無顯著差異;單獨運動介入有助改善生活品質之生理健康範疇。本研究確認運動為社區老人衰弱症狀改善之必要介入策略內容,值得做為未來社區老人延緩失能以預防衰弱之參考。
    Background: Taiwan is expected to be a super-aged society in 2026. Older population is risk group of developing frailty and those older adults with frailty would increase the risk of depression and reduces the quality of life. Therefore, developing appropriate management strategies to improve frailty symptoms, depression symptoms and quality of life in older adults is needed.
    Objectives: To examine the effect of exercise and nutritional intervention on the improvement of frailty symptoms, depressive symptoms and quality of life in the community-dwelling older adults.
    Methods: Eight community care centers were random assigned to four groups: control group, exercise group alone, branched chain amino acid (BCAA) supplement group alone, and a combination of exercise and BCAA supplement. The exercise group alone received 12 weeks exercise trainings. BCAA supplement group alone received daily 5 g BCAA supplement for 12 weeks. The combination of exercise and BCAA supplement received both 12 weeks exercise trainings and daily5 g BCAA supplement. Control group maintained usual care. Those study participants received pre-test and post-intervention test. Measurements comprised demographic characteristics, Fried’s frailty phenotype, geriatric depression scale-15(GDS-15), and the world health organization quality of life (WHOQOL). A generalized estimating equation (GEE) was performed to examine the effect of exercise and BCAA supplement on the improvement of frailty syndrome, depressive syndrome, and quality of life.
    Result: Results showed that the three intervention groups including exercise group alone, branched chain amino acid (BCAA) supplement group alone, and a combination of exercise and BCAA supplement had significant improvement on frailty syndrome compared to control group. Moreover, contrast comparisons showed that the groups of exercise combined with BCAA supplementation group and exercise group alone had higher improvements of frailty syndrome than BCAA supplementation group and control group (p <0.05). The exercise or BCAA supplement did not have significant improvement on depressive symptoms. In terms of quality of life, only the exercise group alone significantly improved the physical health compared to control group.
    Conclusion: The combination of exercise and BCAA supplements and single exercise program intervention exerted similar effects on improving frailty in community-dwelling older adults. Our study highlighted the importance of performing exercise for preventing and managing frailty in community-dwelling older populations. Exercise program would be a critical component of interventions improving frailty syndrome in community-dwelling older populations.
