    Title: 耳針介入對機構長者口腔唾液分泌之改善成效
    The Effectiveness of Auricular Acupuncture on Improving Secretion of Saliva Among Institutional Older Population

    Abstract: 我國老年人口日益增多,入住長照機構長者多受限於身體功能障礙而依賴性高,加上慢性病服用多種藥物的影響,容易引發口乾狀況;對於增加唾液分泌,國民健康署極力推動口腔健康維護,然而,長照機構中對於獨立執行日常生活功能不佳的老人,自行運用按摩唾液腺來增加唾液實為困難。因此本研究主探討採用耳針介入對機構長者口腔唾液分泌之改善成效;研究設計採實驗性研究設計,針對二家機構長者以隨機分配方法分為實驗組和控制組,實驗組老年人接受有效耳穴的耳針照護方案,控制組老年人進行無效耳穴的耳針照護;研究資料包含:人口學變項、一般健康狀況、主觀口腔健康狀況、客觀口腔健康狀況,以耳針介入對機構長者促進口腔唾液分泌,採用視覺量表合併口腔水分計,監測口腔唾液分泌的多寡,研究方式使用卡方檢定、t檢定進行資料分析。研究共召募及介入75位長照機構65歲以上長者,完成研究介入及資料收集共73人;研究結果顯示,二組在主觀嘴唇乾燥無統計顯著意義(p>0.05),然而,實驗組接受耳針介入後較控制組在主觀之說話困難、吞嚥困難、唾液多寡、口腔乾燥、喉嚨乾燥、舌頭乾燥、口渴程度及客觀口腔水分計之唾液分泌檢測均顯著高於控制組(p<0.05),顯示耳針介入有助機構長者促進唾液分泌及改善口乾症狀。本研究結果建議長照機構可考慮採用耳針介入以促進機構長者口腔唾液分泌,及改善口乾症狀。
    A randomized controlled trial was conducted in two nursing homes to recruit eligible study participants. Institutional-dwelling older adults aged more than 65 years olds were randomly allocated to AA group (n=37) or control group (n= 38). AA group received an eight-week auricular acupuncture on the real auricular points: Shenmen, Point Zero, and Sanjiao. Control group received sham auricular point acupuncture. Data collection were at baseline and at eight weeks after intervention. Measurements comprised subjective assessment for level of dry mouth assessed by visual analogue scale xerostomia questionnaire, and objective assessment for the physiological wetness of lingual mucosa measured by moisture-checking device. General estimating equation (GEE) model was performed for data analyses.
    Results: Our findings found that AA group completing 8-week auricular acupuncture at Shenmen, Point Zero, and Sanjiao significant improved subjective symptoms of dry mouth comprising difficulty speaking, difficulty swallow, amount of oral saliva, dry throat, and thirst compared to control group (p<0.001). Moreover, AA group had higher objective oral saliva flow than control group (p<0.001).
    Conclusion: Auricular acupuncture is an effective non-pharmacological method for improving subjective symptoms of dry mouth and objective saliva secretion in older population living in nursing home. Regular performing auricular acupuncture at auricular points of Shenmen, Point Zero, and Sanjiao could be considered as alternative treatment approach for improve dry mouth in institutional-dwelling older adults.
