    摘要: 研究背景:根據聯合國世界衛生組織的報告,預計2050年每6個人中就有1人是65歲以上的高齡者,臺灣在2018年的65歲以上達到全人口的14.56%,正式邁入高齡社會;認知衰弱為可逆之疾病,若能適時發現並預防,可降低失智症發生率,目前已有許多文獻指出,利用體感式遊戲介入,可有效改善長者衰弱及減緩轉為失智症的速度,但目前針對認知衰弱長者之研究仍然稀少。
    研究目的:本研究旨在為探討體感式遊戲介入對認知衰弱長者,提升其身體功能、認知功能及社會心理功能之成效。
    研究方法:本研究使用類實驗性研究,將研究對象分至四個臺北市社區據點,實驗組包含兩個社區據點,共35人及對照組包含兩個社區據點,共34人,以前後測方式進行研究。實驗組進行8週體感式遊戲介入,以團體訓練為主,每週2次,每次40分鐘;對照組則不採任何措施,依照個案本身日常活動進行即可。研究指標包含:(1)身體功能,以客觀測量儀器APDM's Mobility Lab輔助評估身體功能(2)認知功能,以蒙特利爾認知評估量表評估(3)社會心理功能以中文版寂寞量表評估。本研究共收案69位認知衰弱長者,分別為實驗組35人,對照組34人,回收資料運用描述性統計、獨立樣本T檢定、成對樣本T檢定、單變量分析、效果量檢定、皮爾遜積差相關係數作為統計方法進行資料分析。
    研究結果:由統計分析結果可以得知,經由兩個月體感式遊戲介入後,在身體功能的表現上,顯著提升長者其平衡及姿勢控制;應用在認知功能的表現上,顯著提升長者的視覺空間/執行、延遲記憶及整體認知功能表現,但在社會心理功能上無顯著提升。
    結論:體感式遊戲能有效促進認知衰弱長者其身體、認知及社會心理功能;由於目前國內較少針對認知衰弱長者之相關研究,而認知衰弱為高齡領域不可忽視之族群,建議未來研究可基於本研究的結果,廣泛應用於社區據點,且針對不同族群進行更進一步成效探討或制定不同介入方案。若資源充足且政策日趨完善下,亦建議擴大長照服務對象,提供適合資源,以提前預防失智及延緩老化速度。
    Background: According to the United Nations World Health Organization (WHO) report, it is estimated that 1 out of every 6 people in 2050 will be an elderly person over the age of 65. In 2018, Taiwan’s population over the age of 65 reached 14.56% of the total population; Cognitive frailty is a reversible disease. At present, many documents have pointed out that the use of exergaming to intervene in the elderly can effectively improve frailty and the slowing down the rate of dementia, but current research on the elderly with cognitive frailty is still scarce.
    Research Purpose: The purpose of this research is to explore the effects of exergaming on physical, cognitive, and psychosocial functions among the elderly with cognitive frailty.
    Method: This study use a quasi-experimental study to divide the subjects into an experimental group and a control group in different communities in Taipei City. The experimental group will engage in exergaming. The main training is 2 times a week, 40 minutes each time (5 minutes before and after warm-up and cool down and 30 minutes exergames). The total intervention week is 8 weeks. The control group doesn't take any intervention, maintaining their daily activities. The APDM's Mobility Lab was used to assess physical function, the Montreal Cognitive Assessment (MoCA) to assess cognitive function, and the Chinese version of the Loneliness Scale to assess social and psychological functions. Use descriptive statistics, independent samples t-test, paired samples t-test, ANCOVA, power analysis, Pearson product-moment correlation coefficient for data analysis.
    Result: A total of 69 people with cognitive frailty were included in this study. There were 35 people in the experimental group and 34 people in the control group. From the results of statistical analysis, it can be known that the experimental group and the control group are homogeneous before the study (p>.05). After two months of intervention, the experimental group can significantly improve its cognitive function and physical function through exergaming intervention, but there is no significant in psychosocial functions.
    Conclusion: Exergames can effectively promote the physical, cognitive, and psychosocial functions of the elderly with cognitive frailty. So far the evidence of the cognitive frailty is limited in domestic but the cognitive frailty cannot be ignored in the field of geriatrics. It is recommended that future studies can be based on the results of this research and be widely used in the community. In addition, designed different intervention programs on different groups. If the resources are sufficient and the policy becomes more and more complete, it is also recommended to expand the long-term care recipients and provide suitable resources to prevent dementia and delay the speed of aging.
