    題名: 長照需求者之主要照顧者照顧負荷及喘息服務需求之探討

    摘要: 背景:照顧過程中照顧者承受著照顧負荷所帶來的負面影響,過去研究已證實喘息服務能有效降低照顧負荷。目的:本研究期望瞭解長照需求者之主要照顧者其照顧負荷及喘息服務需求。方法:本研究採橫斷式調查研究法,以大台北地區某兩間社區整合型服務中心A級單位所納管的長照需求者之照顧者為研究對象,採面對面訪談、填寫問卷的方式收集資料,收案人數共118人。研究工具分別為照顧者負荷量表(Caregiver Burden Inventory, CBI)進行照顧者照顧負荷之測量,以及研究者參酌相關需求文獻,所改編之「照顧者喘息服務使用之需求調查問卷」做為評估工具。結果:(1)照顧者照顧負荷之單項分析中以「病人依賴著我」的負荷感受最高。(2)喘息服務主觀需求項目當中,照顧者認為需要喘息服務的原因最多為「身體獲得休息,減少健康問題」。(3)經複迴歸分析結果顯示,照顧者自覺健康為預測照顧者照顧負荷得分之重要預測因子。照顧者自覺健康及家庭平均月收入為預測照顧者喘息服務主觀需求之重要預測因子。結論:本研究照顧者皆有明顯的照顧負荷存在,且照顧者對於身體可以休息的需求最多,但卻未曾使用過喘息服務。本研究過程發現照顧者未使用喘息服務的主要原因包括機構式喘息服務之申請方式繁瑣;照顧服務員無法執行侵入性技術,故需照顧者於家中協助照顧等,無法讓照顧者真正達到使用喘息服務之目的;預約困難。部分照顧者亦提出其他未使用的原因有喘息服務核定天數不足、服務內容無法滿足及可使用時間不在範圍內。故建議未來政策加強建構照顧服務員訓練課程及調整薪資,以留住長照人力;建構全台喘息服務線上系統,讓資源透明化,並增加照顧者對服務單位的選擇性;增加喘息服務核定天數;增加喘息服務所提供之內容,如陪同外出、家務協助及備餐服務;規劃提供夜間居家喘息服務,改善照顧者睡眠不足的問題及減輕因照顧所帶來的耗竭。政策規劃若能考量實際情形,喘息服務才得以發揮最大效益。
    Background: The caregivers bear the negative impact of the caregiving burden during the caregiving process. Previous studies have shown that respite care can effectively reduce the caregiving burden. Aim: This study is expected to realize that caregiving burden and respite care in the caregivers of individuals with needs for long-term care. Methods: A cross-sectional survey research was conducted, using face-to-face interviews questionnaires filled out by 118 family caregivers in two care service units located in Taipei metropolitan area. The research tools including the Caregiver Burden Inventory (CBI) were used to evaluate caregiver burden, and Caregiver Respite-Care Needs Questionnaire. Results: (1) The individual analysis showed that the item "He/she is dependent on me" cause the strongest sense of burden. (2) The most common reason for caregivers to demand for respite care was "To get physical rest and reducing health problems." (3) In multiple regression analysis, caregivers’ self-rated health status was a predicting factor for caregiving burden; caregivers’ self-rated health status and the average family income were predicting factors for caregivers’ demand for respite care. Conclusion: Caregivers in this study have obvious caregiving burden, and the most common demand is physical rest, but they have never used respite care. This study found that the main reasons for not using respite care are the application process for institutional respite care is complicated; resident care attendant cannot perform invasive procedures; and difficulty in making appointments. Some caregivers also pointed out that the reasons for their not using respite care include insufficient days, unsatisfactory service content, and available time is out of range. Therefore, it is suggested that future policies should strengthen care worker training courses, and adjust the salary to retain long-term employment; establishing an online system for respite care throughout Taiwan to make resources transparent and increase the selectivity of caregivers to service units; increase the number of approved days for respite care; increase the content provided by the respite service, such as accompany going out, housework assistance and meal preparation; provide night-time home respite care to alleviate the lack of sleep for caregivers and reduce exhaustion caused by care. If policy planning can take the actual situation into consideration, respite care can be most effective.
