    題名: 主治醫師對於醫療專業素養與組織價值觀之契合度探討-以台北市三家醫院為例

    摘要: 在醫院體系中每位醫師扮演著極為吃重的角色,已不再是過去只需將病人醫治好與社經地位那樣高不可攀的專業人士,健康逐漸被視為國民的基本權益。
    過去醫師面對病人問題,多透過專業醫學知識及專精化的技能來解決病人的問題,卻忽略與病患互動及合作,而缺乏對病人全盤的了解。現代醫師的養成,則是強調,除了必備的知識與技能外,也需兼備人文素養與社會意識,如此才能提供以病人為中心的醫療照護。
    本研究係探討在醫療院所之主治醫師的醫療專業素養和組織價值觀,是否與組織內實際狀況的契合度一致,樣本於北市三家區域級以上醫院進行收集,搜集與發放是以委託或親自造訪的方式進行,一共發出700份問卷,並回收148份問卷,扣掉無效問卷後,共計125份有效問卷,並利用描述性分析、獨立樣本T檢定、one-way ANOVA、再利用因子分析與相關性分析進行構面之歸納,最後再依數據、資料與各構面進行分析探討。
    本研究指出不同的組織特性,會影響醫師對於「醫院經營與成長」的實際認同程度,而在不同的任職醫院中,準醫學中心的主治醫師對於「醫療專業知識與技能」、「醫師執業態度」、「維護病人權益」此三構面的醫院實際狀況之認同程度較低。已婚與具管理職的主治醫師對於醫療專業素養擁有較高的認同程度。而主治醫師們當初就讀醫學系的動機與來自不同實習醫療院所對於專業素養的形成並無顯著影響,同時,主治醫師對於醫院實際經營狀況之契合度表現較低。
    此次研究樣本收集係來自之三間醫療院所且為同一體系,擬建議未來研究學者可針對台灣北中南區與不同權屬別之醫院進行研究,同時,國內醫學大學應建立完整的醫學與人文課程,並多給予醫學系學生鼓勵,強化其成為一名專業醫師之動力,及早做準備,面對未來在醫療體系內的人生挑戰時,強化其面對挫折與抗壓力,也才能夠具備高度的人道關懷與濟世救人之精神。
    Medical professionalism has gained raising attention ever since the 1990s. Even has been doubted with conflicting interests, physicians still practice in order to meet social expectation and high standard they set for their career. We, therefore, to set up this research to discover how attending physicians evaluate the importance of medical professionalism. Furthermore, we also are interested in how physicians with managerial positions will react differently in terms of medical professionalism.
    This study defined five medical professionalism (including ‘medical knowledge and clinical skills’, ‘interpersonal skills with patients’, ‘teamwork’, ‘public health duty’, ‘protection of patients’ rights’ ) and three organizational value as eight dimensions (including ‘employee orientation’, ‘profit orientation’ and ‘hospital growth’ ). We modified Chiu’s questionnaires and validated it by experts’ validity check. We issued the questionnaires to 700 physicians who worked in three major hospitals in Taipei area, and 125 respondents finished and returned questionnaires to us. Data was collected from February to May 2016. Data was analyzed by descriptive, bi-variate analysis, and multiple regression.
    This study shows the organzational characteristics significantly associated with physicians’ perceived organzational value. Especially on organizational value of ‘hospital growth’. This study concluded the finding how attending physicians emphasized on the five dimensions of medical professionalism, and physicians with managerial positions reported higher on five dimension. Therefore, the obligation accompanied with managerial positions could play a key important roles to upgrade physicians’ emphasis on medical professionalism.
