    摘要: 馬迴與記憶力及失智症之間有著密切的關係,從微觀的神經細胞生長至神經傳導的活性變化。透過功能性磁振造影的研究,能幫助理解記憶、認知與情緒之間聯繫的腦部網路模式及反應機制。進一步提升成為臨床生物標記應用的可能性,用來輔助臨床診斷,與落實預防醫學的前瞻性研究。本研究計劃目的是運用功能性磁振造影影像分析研究方法來建立 Task-Based 與 Resting-State Functional Connectivity常模來觀察受試者的海馬迴在學習新臉孔與名子配對時,海馬迴活化的程度和功能性連接的變化。同時利用Alzheimer''s Disease Neuroimaging Initiative (ADNI) 資料庫中的靜息態功能性磁振影像 (resting-state functional MRI) 資料,針對人體大腦的預設模式網絡 (default mode network) 與記憶相關的海馬迴功能性網路 (hippocampal functional network) 來探討分析失智症患者的功能性連接變化。
    Dementia is a disturbing, and radical neurodegenerative disease associated with severe memory decline and cognitive deficits. The hippocampus, which is an essential node of default mode network (DMN), has been thought to be involved in memory and emotion processing and were disrupted in patients with dementia. The purpose of this thesis was to construct a comprehensive study by using both task-based functional connectivity MRI (fcMRI) and resting-state (rs-fcfMRI) to evaluate the functional changes and elucidate the heterogeneity within the DMN and hippocampal networks for patients with dementia. The first study aim was to explore the altered functional connectivity in patients with dementia using data obtained from the Alzheimer''s Disease Neuroimaging Initiative (ADNI). Another purpose of this study was to characterize the functional connectivity under a memory task stimulus. Our preliminary results demonstrated the group differences in the inferior frontal gyrus, middle frontal gyrus, thalamus, hippocampus and other areas related to DMN or hippocampal networks in patients with dementia compared to normal healthy controls. This multi-probe functional connectivity approach demonstrates potential for applicability to target the disease-specific biomarkers in advance and to bridge a missing link between early pathophysiology aberrations and neurocognitive declines in patients with dementia.
