Abstract: Background: Cardiovascular disease (CVD) is the main cause of death in the Latin America and the Caribbean region, and coronary heart disease (CHD) is the primary cause in most of these countries. This study aims to assess the level of knowledge of CHD risk factors among Port-Au-Prince adults aged 20-60 years and determine relationship between CHD risk factor knowledge and corresponding health behaviors.
Method: The study was conducted in Port-au-Prince from October to December 2019. It is a cross sectional design and participants were recruited through convenience sampling. Questionnaire for this study was adopted from 3 standardized questionnaires; the World Health Organization (WHO) Stepwise Approach to Non-Communicable Disease Risk Factor Surveillance, the United States of America Centers for Disease Control and Prevention (CDC) Behavioral Risk Factor Surveillance System and the Heart Disease Fact Questionnaire was used to assess CHD knowledge (French version). The score was calculated by summing the correct answer for each item (3-18). Descriptive statistics were performed to determine participant’s knowledge levels, Chi-square test, Fishers test and ANOVA were used for group differences, binary and multinomial logistic regression analyses were performed to determine relationship between knowledge and corresponding health behavior.
Results: 397 participants were recruited with a survey completion rate of 98%. The knowledge mean score was 11.1 (out of 18 questions, 61.9%). Regular physical activity related knowledge and exercise location related knowledge was found to be associated with levels of physical activity with regard to low levels of PA as the reference category. Participants who knew that regular physical activity reducing CHD risk were less like to engage in high levels of PA compared to those who do not knew (OR=0.209; P=.012). Those who agreed that not only exercise performed at gym or exercise class can reduce CHD risk, were less likely to report high levels of PA compare to those who do not agreed (OR=0.432; P=.013). participants who knew that not only exercise performed at gym or exercise class can reduce CHD risk were less likely to reported moderate levels of PA (OR= 0.301; P<.001) compare to those who do not knew. Cholesterol related knowledge was also found to be associated with type of cooking oil used. Participants who knew that eating fatty foods does affect cholesterol level were less likely to report the use of vegetal oil compared to those who do not knew (OR=0.552; p=.034) with regard to none in particular as reference category.
Conclusion: Knowledge was found to be negatively influenced physical activity levels and type of cooking oil among Haitians. However, do not influenced tobacco use status. Enabler interventions such as; making physical accessible in work place and school, making healthy foods affordable and increasing the price of tobacco products are recommended in addition to education program in order to reduce CHD and its associated risk factors in Haiti.
