    摘要: 背景:電腦斷層掃描(CT)的游離輻射在醫學診斷相較於成人,兒童有可能更容易與輻射產生健康風險的主要來源。 方法:本研究從200萬的隨機樣本中使用的樣本,參加者為全國代表性的台灣全民健康保險研究資料庫,包括患者為20歲之前從2000年和2008年之間病患接受至少有一次 CT 掃描。結果:我們在1998- 2011年中自NHIRD (2000年版兩百萬歸人檔)的數據取得的投保兒童(<20歲),排除癌症病史的患者至少兩年(1998年 - 1999年),及滯後追蹤期至少三年(2009年 - 2011年),從2000年 - 2008年的資料中約有20311 CT掃描進行,包括11671(57.5%)的患者為男性,8640(42.5%)人是女性的CT檢查,患者的CT輻射暴露組中有患癌症顯著高出未暴露組(HR=3.39,95%CI=2.52 - 4.55),使用Cox回歸分析與Log-rank檢驗和Kaplan-Meier癌症發病率的生存分析。 討論:流行病學對輻射誘發惡性腫瘤的任何後續風險研究,是非常有價值的,除非小心才會非常有用的,我們要保護民眾的輻射相關性對健康的影響,或許適當的直接觀察會更好,且流行病學研究可以很容易從大的錯誤中得到小的結論、巨大的偏差和低顯示的統計效果。在這項研究中沒有相關經典的癌症因子分析,如高血壓、肥胖、吸煙和任何癌症相關的疾病循環系統,但我們期望和理解游離輻射的相關癌症疾病影響特別在於低劑量的暴露。
    BACKGROUND: Computed tomography (CT) is a major source of ionizing radiation exposure in medical diagnostic. Compared to adults, children are supposed to be more susceptible to health risks related to radiation. METHODS: This study used information from a random sample of 2 million persons enrolled in the nationally representative Taiwan National Health Insurance Research Database. Exposed cases consisted of patients with before 20 years old who underwent at least one CT between 2000 and 2008. RESULTS: We obtained data of insured children (< 20 years old) from NHIRD (2000 ver. – two million cohort) in 1998 – 2011, exclusion cancer history patients minimum two years and follow up minimum three years until 2011, from the cohort in year of 2000 – 2008 approximately 20311 CT scans were performed, included 11671 (57.5 %) patients were male, and 8640 (42.5 %) of whom were female CT examination, Patients in the CT radiation exposed group had a higher risk of developing cancer (HR = 3.39, 95% CI = 2.52 – 4.55), and the exposed group had significantly higher by Cox-regression with Log-rank test and Kaplan-Meier survival analyses of cancer incidence. DISCUSSIONS: Epidemiology is invaluable for study radiation-induced any subsequent risk of malignant tumor, they are very useful and unless careful, but better if direct observation of health effects of relevance by complemented in populations that we want to protect, epidemiology can be easy subject to small effects from big error, huge bias and low statistical power. In this study did not classical cancer factor such as hypertension, obesity, smoking and any cancer related disease, but we expect and understanding of the related disease circulatory with ionizing radiation specially exposed at low doses.
