    題名: 職場霸凌對護理人員照護病人的安全態度及安全績效之影響
    The Effects of Workplace Bullying on Patient Safety Attidutes and Safety Performance for Patient care in Nurse

    摘要: 隨著時代的變遷職場件事霸凌有越來越多的趨勢,相關研究指出霸凌對受害者會造成心理壓力和身體傷害,會增加請求調單位的機會,缺乏責任感及降低照護品質,進而影響護理人員照護病人的安全態度與安全績效,本研究之目的在探討職場霸凌對護理人員照護病人的安全態度及安全績效的影響。本研究為橫斷性問卷調查研究,採立意取樣以某區域教學醫院的護理人員為研究對象,運用Soper的計算所需樣本數387,研究工具為結構式問卷,包括「個人基本資料表」、「負向行為事件量表」、「病人安全態度量表」及「安全績效量表」四部分,資料收集後以SmartPLS2套裝軟體進行資料分析。
    研究期間從2014年5月10日至6月30日共發出問卷420份,回收402份,剔除填寫不完整問卷11份,納入分析之有效問卷共391份,有效問卷回收率93.09%。研究發現護理人員有29.8%的人表示曾經遭受不同程度的職場霸凌。對病人的安全態度得分為3.58(SD=0.54)和安全績效得分為3.79 (SD=0.56),均趨近中等程度,介於中立和有些同意之間。職場霸凌對護理人員照護病人的安全態度的路徑係數 (β=-0.47, p<.001)有顯著負相關,而在護理人員照護病人的安全態度對於安全績效路徑係數 (β=0.71, p<.001)有顯著正相關,但職場霸凌對護理人員照護病人的安全績效路徑係數(β= 0.02)未達顯著影響,經由SmartPLS顯示護理人員照護病人的安全態度是職場霸凌對安全績效的完全中介因子,且sobel test達顯著(p<0.001)。
    本研究結果提供行政管理者,瞭解職場霸凌對護理人員照護病人的安全態度及安全績效的相關性,並做為行政領導者及管理者之參考,期能提升護理人員照護病人的安全態度及安全績效。
    Overtime, workplace bullying shows a growing trend. Researches have pointed out that bullying can mental stress and physical injury on victims. In medical institutions, bullying may result in job transfer, loss of sense of responsibility, reduced care quality, poor attitude and performance, which may endanger patients’ safety. The purpose of this research is to explore the impact of workplace bullying on nurses’ attitude and performance toward patient safety.This is a cross-section survey research upon purposive sampling, with nurses from a regional teaching hospital as subjects. Soper’s calculation found that a sample size of 387 is required. The research tool is a structured questionnaire with four parts, including Basic Personal Information, Negative Behavior Scale, Patient Safety Attitude Scale, and Patient Safety Performance Scale. SmartPLS2 was used to analyze the collected data.
    From May 10, 2014 to June 30, 2014, the researcher distributed 402 questionnaires and retrieved 402 samples. After eliminating 11 invalid samples, there were 391 valid samples, with a valid return rate of 93.09%. The analysis found that 29.8% of nurses have suffered various degrees of workplace bullying. The scores of nurses’ attitude and performance toward patient safety are 3.58 (SD = 0.54) and 3.79 (SD = 0.56) respectively. Both are close to moderate degrees, ranging between neutral and somewhat agree. The path coefficient (β = -0.47, p < .001) of workplace bullying to nurses’ attitude toward patient safety has a significant negative correlation. The path coefficient (β = 0.71, p < .001) of nurses’ attitude to performance toward patient safety has a significant positive correlation. However, the path coefficient (β = 0.02) of workplace bullying to nurses’ performance toward patient safety has no significant impact. The SmartPLS found that nurses’ patient safety attitude is a full mediator of workplace bullying on safety performance, and its p-value of Sobel test has significant impact (p< 0.001).
    The findings can serve as a reference to help hospital administrators understanding the correlation between workplace bullying and nurses’ attitude and performance toward patient safety, in order to improve nurses’ attitude and performance toward patient safety.
