    摘要: 本研究乃對於不同表面功能化處理的牙科植體進行臨床前及臨床研究。局部缺牙或是全口無牙之患者,採用牙科植體作為輔助的治療方式,已是相當普及的治療考量之一。近三十年來,牙科植體的演變過程相當迅速,為了符合生醫材料所應具備的生物相容性、抗腐蝕性與適當的機械強度等要求,所使用的植體材料也從黃金、白金、陶瓷等,逐漸趨向目前市面上各方所具共識的鈦金屬。為了達到理想的骨整合效果,植體的表面處理模式則成為下一階段研究的目標與方向。
    誠如上述,我們以不同表面功能化處理的牙科植體應用於動物實驗及人體實驗上,進行探討骨整合情形;表面功能化處理技術包含噴砂,噴砂後酸蝕,雷射處理,創新型電化學處理,及血液幹細胞處理等技術應用於牙科植體上。期望經動物實驗及人體實驗能找出最有利於骨細胞生長之表面功能化處理技術,以期日後應用於牙科植體之改良,增進骨整合與臨床表現之效果。
    The aim of the present study was to examine the early osseointegration of implants containing various surface functionalizations such as machined, sandblasting acid etching (SLA), plasma discharge laser (PDL), and SLAffinity treatments. In addition, the implants contained StemBios (SB) cell was also performed to study on preclinical and clinical trial. The implant surfaces with various surface functionalization were characterized using scanning electron microscopy (SEM), atomic force microscopy (AFM) and transmission electron microscopy (TEM). Material analysis was also performed clearly. For the clinical success of dental implant treatments, it is important to evaluate the magnitude and location of the maximum stresses. Stress analysis was performed using 3-D models constructed using a computer-aided program. Six patients were enrolled in a clinical trial. Each patient received one SLAffinity implant in the mandible. Of these six patients, three received SB cell therapy in addition to the SLAffinity-treated implant. Resonance frequency analysis (RFA) and computed tomography (CT) were performed at 3 time points; 1, 2, and 4 months after implant treatment. Our results indicate that the additional SB cell therapy significantly promotes early osseointegration, especially in the early stages of bone tissue remodeling. SB cell therapy also facilitates efficient stress transfer in SLAffinity dental implants. SLAffinity implants proved to be a clinical success over the 4-month period as well.
