    題名: Konus 雙重冠活動義齒結合有限元素分析之生物力學研究
    Biomechanical Research of Konus Telescope Removable Denture Stress Distribution by Finite Element Analysis

    貢獻者: Konus telescope removable denture is composed of inner crown and outer crown as retainer. Double crowns were using the friction force or wedge effect to tight the removable denture and the abutment tooth. The decision of konus denture retentive force, which the proper force is between 500~900 g, was determined by konus taper angle of inner crown. To avoid denture separate by adhesive food, or avoid abutment tooth hurt by excessive force, the optimal retentive force is 2~3 kg, which means 3~5 abutment tooth. Usually, the optimal konus taper angle is 6 degree in clinic. Sometime, consider with the viability of abutment tooth, we would choose 4~8 degree to adapt the denture retentive force. In this study, we use finite element analysis to simulate the stress distribution of the periodontal tissue when the occlusal force happened on the konus telescope removable denture. We reconstructed the jaw bone model and premolars model with CT image, and used CAD software to build the konus inner crown, outer crown, implant and abutment, and combined together into a finite element model. Then, use finite element analysis to simulate konus telescope denture when the occlusion forced, and get result of the stress distribution of the bone tissue. In conclusion, the degree and length of inner crown don’t influence the stress of biting force on implant or bone tissue, but the length of outer crown do. Then, with the communication between the dentistry and technician, the patient can get a perfect implant prosthetic and increase the implant success rate.

    摘要: 雙重冠(konus)義齒是指以內冠及外冠所構成的雙重金屬冠為固位體的義齒,
    為利用內外冠的緊密適合所產生的摩擦力或楔子效果來維持。Konus 義齒的維持,
    理論上是由konus 角的大小來決定,適當的內外冠維持力一顆支柱牙約為500-900
    g,平均為700 g,因此合計在2-3 kg 最好,這個維持力在義齒使用時不會因黏著
    性的食物而發生脫離,而且不會對病人的支柱牙發生太大的受力及外傷性變化。
    konus 的角度,通常以6 度最為理想適當,但是在臨床的實際上,考量支柱牙的
    骨植條件狀態或動搖度,konus 角在4-8 度間變化,在骨植狀態不良的支柱牙把
    角度稍微加大來調整義齒整體的維持力。
    本研究利用有限元素分析法分析konus 雙重冠活動義齒受咬合力時牙周組
    織所產生之應力,將CT 影像重建之下顎骨模型及小臼齒模型,並利用CAD 軟
    體將konus 內冠、外冠、植體、支台體等一併組合成有限元素分析模型,模擬
    konus 雙重冠義齒受咬合力時,對於骨組織所產生之應力。經由有限元素分析的
    輔助,可歸結konus 內冠角度與長度對於單顆義齒承受咬合力時並無顯著影響,
    僅外冠長度對於骨組織及植體會造成較顯著的差異,而konus 雙重冠義齒能成功
    地運用於臨床,仰賴醫師與技師之間的協調與配合,多起臨床病例亦證明konus
    雙重冠確實可增加植體贋復的成功率。
