    摘要: Mechanical properties are important parameters for disease diagnosis because they are closely related to the tissue pathological conditions, e.g. cardiovascular disease, atherosclerosis and cancer/tumor. In the past, doctors diagnose these diseases all depends on palpation, which is not accurate and the opinions may change from person to person. Recent studies underscores the importance of developing new technologies including optical coherence tomography (OCT) in addressing challenges including the disadvantages of currant modalities with lack accuracy in the quantitative information, low spatial resolution and inconvenient of differentiating aggressive from relatively indolent lesions. Optical coherence elastography (OCE) is a variation of OCT which is capable of providing quantitative information regarding the stiffness of tissue. OCE could retain its ultra-high resolution (up to 5 μm) whilst it is a non-invasive imaging modality. It could be highly practical in clinical application due to its ability to characterize soft tissue.
    In this research, we built up a system utilizing OCE for measuring the elasticity property of human ex vivo prostate biopsies. The findings of OCE then further used to differentiate aggressive from indolent prostate cancer (PCa). This is further developed to estimate the volume, differentiate and manage different grades and stage of tumor. After collected and analyzed data from prostate tissue, we cross validate the result with the golden standard histology result and thus evaluate the system accuracy. The result shows the highly appropriateness of OCE as a prostate detection modality. The higher value of Young’s modulus obtained from lesion has worse prognosis. Moreover, due to the multifocal property of prostate cancer, the diseased ratio in a biopsy is also valuable information. The system manufactured in this study also has the ability to measure the percentage of abnormal tissue. The sensitivity and specificity of this diagnosis approach is confirmed by varies static analysis. And also, all of the verification indicated the great accuracy of this system.
