    摘要: 由於白內障目前為可治療疾病中,導致失明佔第一位者,因此本試驗乃是針對雙眼老年性白內障患者,無合併其他眼科病變者所進行之研究。而人類眼角膜內皮細胞密度,自出生後即隨著年齡增加會逐年降低,也會受到許多外來因素導致破壞,當細胞密度低於每平方厘米五百顆細胞時,角膜內皮細胞之排水生理功能將無法發揮,並進而導致角膜水腫混濁。由於人類角膜內皮細胞在體內具有不會分化生長的特性,故如何保護其不受到傷害或是讓傷害減到最低,實屬一重要議題。此外,超音波技術應用在醫學上也有一段時間,而眼科白內障手術使用超音波之技術也有十多年,但超音波在進行的同時會產生震波與熱能,也會造成其他組織的受損,故近年來持續有新的超音波技術在發展。因此,本研究對象主要為具有雙眼老年性白內障患者,並願意接受白內障手術並人工水晶體植入手術與研究追踪者,本研究主要使用前瞻性研究的方式收案,樣本取自於台北醫學大學市立萬芳醫學中心,接受白內障手術者,隨機選取使用旋動式超音波系統(厄齊爾系統)或是傳統超音波系統,並探討接受白內障手術的旋動式超音波模式使用時間與傳統超音波系統使用時間對角膜內皮細胞損傷與視力品質之相關性。收案時間為2007年10月起至2008年05月底。
    Purpose : To compare the intra-operative outcomes of cataract
    surgery which are performed with torsional mode and
    conventional ultrasound mode by using the Infinity
    Vision System
    Methods : In this randomized study, 80 eyes were assigned to
    phacoemulsification by torsional mode or
    conventional ultrasound mode. The surgery was
    performed by an experienced surgeon, and the
    outcomes was evaluated by an examiner who was
    masked to treatment assignments.
    Results: Postoperatively, no significant differences were found
    among the groups. However, there was a significant
    difference (P <0.05), in BCVA at day 1,day7 and day
    30 after the surgery compared with baseline in
    both groups. There was a 5.8% and 9.8% loss of
    endothelial cells in the Ozil system and ultrasound
    group, respectively.

    Conclusion: Visual acuity, pachymetry, and endothelial cell loss
    after cataract extraction and intraocular lens
    implantation using a Ozil system is similar to
    conventional ultrasound.
