    摘要: 在腸胃道癌症及婦女的癌症中,腹膜內癌轉移在癌症末期是一個相當常見的病徵,使用全身性化學治療通常會有較差的預後,細胞減積手術合併腹膜內熱化學治療來治療腹膜內癌轉移病患可比傳統全身性化學治療提高存活期。使用阿黴素的治療是很多腫瘤學療法的基礎;不過,在臨床上因為心臟毒性的關係而對阿黴素有使用限制。微脂體劑型的發展可以減少心臟暴露於阿黴素的毒性甚至可以增進藥物在腫瘤組織的釋放。
    本篇研究目的期望能探討在小鼠腹膜內癌轉移動物模式中伴以腹膜內熱化學療法後的療效中使用阿黴素或微脂體阿黴素之比較。
    在本篇研究中,體外實驗我們證實了熱可以增加阿黴素進入細胞的濃度。在體內實驗中,我們在小鼠上建立了一個穩定的腹腔內熱化學治療法來治療腹膜內癌轉移動物模式,其結果證實有微脂體包裹的微脂體阿黴素在此動物模式中比阿黴素有更佳的療效,本篇研究結果顯示在腹腔內熱化學治療法上或許微脂體阿黴素是另一項新的選擇。

    Peritoneal carcinomatosis is a common sign of advanced tumor stage, disease progression or recurrence in numerous tumor entities of gastrointestinal or gynecological origin, treated with systemic chemotherapy alone with very poor response, complete cytoreductive surgery plus hyperthermic intraperitoneal chemotherapy (HIPEC) is effective in the prevention and treatment of peritoneal carcinomatosis. Infusional therapy with the doxorubicin is the cornerstone of many oncological regimens; unfortunately, however, clinical use of doxorubicin is limited by dose-related cardiotoxicity. Pegylated liposome were developed to diminish cardiac exposure to doxorubicin while also preserving or even improving doxorubicin delivery in tumors.
    Purpose of this study hope to explore the efficacy comparison between pegylated liposomal doxorubicin and doxorubicin in the peritoneal carcinomatosis mouse model using hyperthermic intraperitoneal chemotherapy.
    In our research,in vitro study, we demonstrated that heat can increase the delivery of doxorubicin into tumor cells.In vivo study, we built a stable system using HIPEC for the treatment of peritoneal carcinomatosis in mouse model.The result suggested that the pegylated liposomal doxorubicin is more efficacy than doxorubicin,the results of this study appear that may liposomal doxorubicin is another new option in hyperthermic intraperitoneal chemotherapy.
