    本研究的目的在瞭解醫療機構品管圈圈成員對品管圈活動成效之看法與進行圈活動所遭遇的困難,並嘗試探討影響對成效與遭遇困難之相關因素,並根據研究分析的結果對推行品管圈的醫療機構與負責醫品圈競賽的機構提出建議。
    本研究為橫斷性研究,針對醫品圈競賽活動的第四屆全部參賽者為調查對象,以結構式問卷為測量工具,內容包括品管圈推行的主觀成效看法18題項與遭遇的困難24題項,以5分尺度量表評價「同意程度」,並嘗試分析參賽者的個人因素、所屬機構推行模式與成效看法、遭遇困難的相關性。問卷以郵寄方式進行,並有各機構連絡員協助回收,發出700份問卷,回收458份,回收率65.45%,主要分析方法以描述性統計、雙變項分析,來了解影響品管圈活動成效與困難的因素,再以迴歸分析探討影響變項中何者可預測成效與困難,及其預測力如何。
    458位研究對象的整體成效看法平均值3.64,排序前五名的成效有四題項是「個人成效」構面,分別是「提供自我學習與成長機會」「增進品管知識與技巧」「提高對問題的警覺度與改善意識」「提昇問題解決能力」。遭遇困難的整體平均值2.88,最困難的五題項「編寫成果報告費時」「現場發表壓力大」「資料蒐集整理費時」「醫師參與度低」「個人原本工作負荷重,無法增加工作」都屬於「QCC技能」構面。
    圈成員的職位別、服務年資、圈角色、參加動機、目前是否組圈、是否有繼續組圈意願、接受教育訓練時數與主管是否支持均會影響品管圈活動成效的看法。在遭遇困難方面的影響因素則有參賽組別、職位別、組圈動機、目前組圈與否、繼續組圈意願、教育訓練時數及機構是否有專責單位、內部輔導員、講師培訓及主管是否支持。迴歸分析結果,成效看法不同構面的重要影響因子有:繼續組圈意願、接受教育訓練時間、教育程度、服務年資、主管支持與是否有專責單位。在遭遇困難不同構面的重要影響因子有:參加圈活動動機、參賽組別、繼續組圈意願、是否有專責單位、是否有講師培訓、獎勵制度、是否有內部輔導員。

    The purpose of this study is to understand participants’ point of view toward efficacy of QCC and encountered obstacles in the process of QCC activities. Also trying to explore the related factors and coming out some suggestions to both healthcare facilities that promoting QCC activities and institution that hosted the campaign according to the results.
    This is a transverse study. It is using a constructional questionnaire to survey all the participants of the 4th Healthcare Quality Improvement Circle Campaign hosted by TJCHA. The tool is subjective opinion and using 5 point score. It includes basic information, 18 questions regarding the efficacy and 24 questions regarding obstacles. The 700 questionnaires were sent out by mail and received 458 copies back. The returning rate is 65.45%. The major analysis methods are descriptive statistic, 2 way variance analysis and multiple regressions.
    The view of efficacy score mean is 3.46.The four of top 5 items which belong to personal gain are “offering self-learning and growing opportunities”,” increase knowledge and skill of quality control”, “heighten alert to problem and consciousness of improvement” and “promote problem-solving ability”. The mean of encountered obstacles score is 2.88. The first 5 obstacles are “writing the report is time consuming., “high presentation stress”, “data collection and organization need a lot time” “ low physician participation” and “high original workload”.
    There are a couple of factors affect the view of efficacy. They are ‘member’s title’‘work experience’,‘the role in QCC’‘ attending motivation’’ current status of attending QCC’‘will to continue’’training hours’and ‘leader’s support’. The factors related to the encountered obstacles are ‘contest category’‘title’’ attending motivation’‘will to continue’’training hours’ and ‘in charge unit, ‘instructor training inside the institution’‘lecturer training’ and ‘leader’s support’. The regression analysis results show that important factors to influence the view of efficacy are ‘will to continue’’ training hours’‘education’ and ‘work experience’‘leader’s support’‘in charge unit, The important factors to influence the encountered obstacles are, ’ attending motivation’ ‘contest category’‘will to continue’‘in charge unit, instructor training and ‘incentive system inside the institution’ and ‘lecturer training’.
