摘要:	中文摘要背景 : 根據國際糖尿病聯盟(International Diabetes Federation, IDF) 2013年的資料顯示，糖尿病在全球盛行率及發生率逐年攀升，截至2013年全球超過3.82億的糖尿病病患，台灣則最少有超過兩百萬的糖尿病病患，每年全球花費超過5890億美元的醫療費用在糖尿病及相關疾病上，隨著台灣糖尿病病患越來越多，龐大的醫療費用逐漸成為健保資源的重大負擔，高血糖不但導致血管病變以及器官病變更直接影響病患的生活品質及造成龐大的醫療費用，因此有效協助病患控制血糖已成為醫院及醫界不可忽視的重大任務。目的 : 以健保資料庫進行長期追蹤探討台灣糖尿病病患的糖尿病病患病程及預後。研究方法 : 本研究使用健保資料庫2010年版百萬歸人檔資料，選取2003~2008年的糖尿病病患，排除不符合本研究的對象後，每個病患追蹤5年，使用SAS9.3版進行描述性及推論性統計，描述性以平均數及標準差、樣本數與百分比呈現，推論性統計使用Cox proportional hazards model、Kaplan-Meier、Log-Rank-Test進行檢定探討自變項對於糖尿病病患病程及預後的影響。結果及討論 : 本研究發現醫師對不同年紀的糖尿病病患給藥會有所差異。加入糖尿病試辦計畫對於糖尿病病患的病程及預後都較優。內分泌科專科醫師開立胰島素的比率顯著高於其餘的專科醫師。關鍵字 : 第二型糖尿病、降血糖藥物、胰島素、中風、心肌梗塞、健保資料庫。 Abstract Background: According to International Diabetes Federation (2013), the prevalence of diabetes is rising. In this entire world, there are more than 0.3 billion diabetic patients which costing more than 598 billion US dollar every year. In Taiwan, there are more than 2 million diabetic population. With the increase in diabetic patient, the medical expense is becoming a burden to Taiwan NHI system. Therefore, controlling patients’ the blood glucose is one more the priority task for healthcare system. Purpose: Using NHI database to explore the duration of sickness and recovery state of diabetic patient. Method: This study used NHI database 2010 version which select DM patient from 2003 to 2008. Each entries were followed up for 5 years. SAS 9.3 were used for analysis. Conlusion: This study discovered that there was a significant association between drug administered and age of the patient.
