    背景:為提升醫療院所醫療作業資訊化及病歷電子化,提升醫療照護品質,促進院際病歷互通整合,減少病人重複用藥、檢驗及檢查,提升醫療資源運用效能,發展電子病歷已成為台灣現階段推行的重點。本研究目的為瞭解影響基層診所醫師使用電子病歷的滿意度之相關變項,並進一步探討基層診所醫師使用電子病歷的重要性及滿意度之關係。
    方法:本研究為橫斷式量性研究,研究對象為參與行政院衛生署2010年「加速診所實施電子病歷推廣案」所完成之1044家向衛生局所報備實施電子病歷之西醫診所。研究工具為自填式結構性問卷,經信度及效度檢定後,以郵寄方式寄發至診所,有效問卷為556份,回收率為53.77%。
    結果:在整體滿意度方面,發現每月門診量為1000-1999、2000-2999及3000人以上(p<0.001;p<0.001;p=0.002)對基層診所醫師使用電子病歷滿意度有顯著的正向影響;成本效益(p<0.001)、知覺易用性(p<0.001)、主觀規範(p<0.001)、安全(p=0.019)與使用意願(p=0.01)對基層診所醫師使用電子病歷滿意度有顯著的正向影響;而隱私有顯著的負向影響(p=0.001),此模式可解釋49.9%之變異量(Adjusted R2 =0.499)。基層診所醫師對其原有醫療資訊系統廠商之「門診系統功能滿意度、門診系統廠商服務滿意度」越高,對本研究所列各評估構面均有越正向之反應。
    結論:成本效益、知覺易用性、主觀規範、安全、隱私與使用意願對基層診所醫師使用電子病歷滿意度有顯著影響;基層診所醫師對於目前健保費用的抽審與申報作業,感覺很重要但並不滿意,期待政府相關單位提供更簡易、更迅速之電子化之審查作業方式或資訊系統,將更有助於提升基層診所使用電子病歷之意願。

    Title of Thesis: A Study on the Electronic Health Record with Primary Care Physicians' Perspective Through Importance-Performance Analysis

    In this study, a cross-sectional quantitative study, the subjects to participate in the clinics of the Department of Health in 2010 "to accelerate the implementation of electronic medical records to promote the case completed by 1044 which report to the Health Bureau, the implementation of electronic medical records of clinics. Research tool for self-administered structured questionnaire, the reliability and validity test, sent by post to the clinic, valid questionnaires to 556, the recovery rate of 53.77%.
    Cost-effectiveness, perceived ease of use, subjective norms, security, privacy and intention to use the clinic physician satisfaction with the use of electronic medical records have a significant effect. Primary care physicians for the current National Health Insurance of Examining and reporting operations, primary care doctors are feeling very important, but not satisfied and look forward to the Government relevant units easier and more rapid electronic review of the practices or information system, would further contribute to enhance the willingness of primary care clinics using electronic medical records.
