    摘要: 根據衛生署 2011 年統計顯示糖尿病及腎臟疾病於國人十大死因為第四及第十位,並且2012年美國腎臟資料登錄系統 ( United States Renal Data System, USRDS )統計資料顯示台灣末期腎病 (End-stage renal disease, ESRD)盛行率為全球之冠,其中以糖尿病所引發之腎病變為末期腎病主因之ㄧ。文獻指出長鏈n-3脂肪酸補充劑可延緩腎病進展,研究中發現以n-3長鏈脂肪酸介入糖尿病患者發現與降低尿蛋白排出量具有相關性,而流行病學文獻指出補充n-3長鏈脂肪酸對於預防及延緩慢性腎病仍具爭議。本研究目的以食物攝取頻率問卷觀察糖尿病腎病變患者與長鏈n-3脂肪酸及魚類攝取量,並探討上述攝取量對腎功能指標之影響。結果顯示:腎病變組受試者低脂、中脂、高脂魚類攝取量顯著低於糖尿病組,而n-3長鏈脂肪酸攝取量兩組間無顯著差異,而高脂魚類攝取與腎功能指標中肌酸酐呈現負相關 (r= -0.168, P= 0.044)。邏輯斯迴歸分析顯示低脂、中脂及高脂魚類對糖尿病腎病變之勝算比分別為0.768 (P= 0.029),0.743 (P= 0.014)及0.733 (P= 0.006),表示增加低脂、中脂及高脂魚類攝取有助降低罹患腎病變風險。結論:經由膳食問卷調查分析魚類攝取量與降低糖尿病腎病變罹病風險具相關性。
    According to 2011 statistics issued by the Department of Health, diabetes (ICD-10 E10-E14) and kidney disease (ICD-10 N00-N07, N17-N19, N25-N27) are the 5th and 10th pick among the top 10 ranking causes of death.
    The literatures have indicated that long-chain n-3 fatty acid supplements can delay kidney disease progression, some studies found a relationship between long-chain n-3 fatty acids supplementation and decreased proteinuria in diabetic patients However, epidemiological literatures about the supplementation of long-chain n-3 fatty a cids for the prevention and delay of chronic kidney disease are still controversial. The purpose of this study was to observe the intake of long-chain n-3 fatty acids and fish in patients with diabetic nephropathy by food frequency questionarire and to investigate the impact of these intakes on renal function.
    The results showed that nephropathy group had lower intakes of low-fat, moderate- fat and fatty fish than did diabetic group. There was no difference in long -chain n-3 fatty a cid intake between two group. The results show that the intake of fatty fish and creatinine showed a negative correlation (r = -0.168, P = 0.044). Regression analysis showed that odds ratio of low-fat, moderate-fat and fatty fish intakes were 0.768 (P = 0.029), and 0.743 (P = 0.014) and 0.733 (P = 0.006), respectively, showing that an increase in low-fat, moderate-fat and fatty fish intakes will reduce the risk of diabetic nephropathy. In conclusion, fish consumption may be asso ciated with a decreased risk of diabetic nephropathy.
