    摘要: 非酒精性脂肪肝疾病 (non-alcoholic fatty liver disease, NAFLD) 為常見的慢性肝臟疾病。本實驗使用高油液態飲食誘發大鼠 NAFLD,給予大豆蛋白或 b-conglycinin 取代實驗飲食中部分蛋白質,探討大豆蛋白延緩 NAFLD 的疾病進展是否為其中的 b-conglycinin,並進一步探討 b-conglycinin 對 NAFLD 影響的機制為何。實驗為期 24 週,前 16 週為誘發期,依體重隨機分成兩組,控制組 (C group, n=8) 給予控制飲食而 NAFLD 組 (N group, n=32) 則給予高油液態飲食,於第 16 週時犧牲部分實驗動物,確認是否成功誘發 NAFLD。結果發現給予高油液態飲食後,可觀察到體內出現胰島素阻抗、肝臟脂肪堆積以及肝臟發炎的情形,確認成功誘發 NAFLD。接著將其餘 24 隻大鼠,依體重和肝功能指標隨機分成三組,NAFLD 組 (N) 持續餵食高油液態飲食,而 NS 組和 NB 組則分別以 50?s 的大豆蛋白和 12.5?s b-conglycinin 取代高油液態飲食中的酪蛋白,進行為期 8 週的實驗。實驗結果發現給予大豆蛋白和 b-conglycinin 可顯著降低大鼠肝臟三酸甘油酯濃度和血漿中 leptin 的濃度,且經由病理切片結果也發現可改善肝臟脂肪堆積,亦可顯著降低肝臟中 cytochrome P4502E1 (CYP2E1) 蛋白質表現量,進而減緩肝臟發炎的情形。此外,大豆蛋白可減少腸道菌相中有害菌的數量,延緩體內發炎反應的發生。依此推論,大豆蛋白可藉由改善肝臟脂肪堆積和肝臟發炎的作用,進而延緩 NAFLD 的疾病進展,此作用可能來自於大豆蛋白中所含的 b-conglycinin。
    Non-alcoholic fatty liver disease (NAFLD) is a common cause of liver disease. Therefore, the aim of this study was to investigate the mechanisms and effectives components of soy protein on NAFLD. In this study, the rats were fed a control diet or high-fat diet for 16 weeks to induce NAFLD and then were distributed into one of three diets: a NAFLD-inducing diet, a NAFLD-inducing diet plus soy protein isolate or ??-conglycinin as substitution of part of casein for 8 weeks. We found that soy protein and ??-conglycinin significantly decreased liver triglycerides and plasma leptin concentrations. Furthermore, soy protein and ??-conglycinin lowered hepatic CYP2E1 protein expression and inflammatory substances levels. In addition, soy protein intake modified gut flora. In conclusion, soy protein may ameliorate the progression of NAFLD by lowering liver fat accumulation and inflammation, and the effect may through its b-conglycinin component.
