    摘要: 無機砷暴露無法直接自人體排泄出來,所以人體暴露無機砷後需要一系列還元反應與甲基化反應來代謝和解毒經由尿液排出。許多因素與人體砷甲基化能力好壞有關。然而目前只有少數人體砷甲基化能力相關因素的研究,特別是在兒童和青少年。砷暴露可能跟胰島素阻抗有關,但目前兒童和青少年的研究有限或不確定。因此,本研究的目的擬探討無機砷甲基化能力,肥胖和胰島素阻抗之間的關聯。
    這項研究收集279名小學生和301名國中生,所有參與者在簽屬同意書後進行問卷訪視和體位測量,並收集尿液與血液樣本,進行尿液砷物種分析和血液生化檢驗。利用高效能液相層析儀,連結氫化物發生器和原子吸收光譜儀測定尿中三價無機砷、五價無機砷、五價單甲基砷酸和五價雙甲基砷酸的濃度。
    結果發現國中生比國小生有較低的身體脂肪百分比,較低的父親抽菸狀態,父母的教育水準較低,低膽固醇與五價雙甲基砷酸;但有較高尿液肌酐酸,尿液總砷含量,三價無機砷,五價單甲基砷酸與無機砷百分比。尿液砷物種在不同身體質量指數組別無顯著統計上的差異。在多變項調整分析後,尿液砷含量(?慊/g creatinine)和無機砷,與HOMA-IR值呈現顯著正相關。綜上所述,砷暴露與兒童和青少年的胰島素阻抗有相關性。但肥胖影響砷甲基化能力和砷暴露影響胰島素阻抗的機制仍然需要進一步調查。
    Arsenic ingestion could not be excreted directly in human. Individual exposure to inorganic arsenic (InAs) needs a series of reduction and methylation reactions to metabolize and detoxify and then excretion through urine. Many factors have been known to be related with arsenic methylation capacity. However, less research was performed to identify the related factors of arsenic methylation capacity, especially among children and adolescents. Besides, chronic arsenic exposure is associated with insulin resistance, but the research is limited or inconclusive among children and adolescents. Therefore, the aim of this study is to explore the association between inorganic arsenic methylation capacity, obesity, and insulin resistance among children and adolescents.
    This study collected 279 elementary students and 301 junior high school students. Questionnaire interview and anthropometric measurements were conducted for all participants after their informed consents were received. Spot urine samples and the peripheral blood specimens were collected for urinary arsenic species analyses and biochemical test. The urinary concentrations of iAs III, iAs V, MMAV, and DMAV were measured by high-performance liquid chromatography (HPLC), equipped with a hydride generator and atomic absorption spectrometer (HG-AAS).
    Present study found that less body fat percentage, more frequency of cigarette smoking status of father, lower educational level of parents, higher urine creatinine, lower cholesterol, higher urinary total arsenic level, higher arsenite, MMA, InAs percentage, and lower DMA percentage in students of junior high school than elementary school students. There is no significant difference of urinary arsenic metabolites among different BMI groups. Urinary arsenic level (?慊/g creatinine) and arsenite level were positively related with HOMA-IR level after multivariate adjustment.
    In summary, arsenic exposure was correlated with insulin resistance in children and adolescents. But the mechanism of obesity influence the arsenic methylation capacity and arsenic exposure results to insulin resistance will be needed further investigation.
