    摘要: 急性胰臟炎會導致全身性發炎反應,在臨床上也會引起嚴重的併發症,一般認為免疫風暴在急性胰臟炎的病生理學上扮演重要角色。由多形性白血球所媒介之多重器官衰竭需要分解細胞基質,因此基質金屬蛋白酵素參與多形性白血球的組織侵犯是合理的推論。
    在本研究發現,多劑量的腹腔注射cerulein藥物可以誘發小鼠產生急性胰臟炎。更進一步,本研究建立一種新的動物模型,模擬急性胰臟炎併發腹腔感染。另一方面,探討基質金屬蛋白酵素在各種有無合併感染的急性胰臟炎動物模型上的表現,結果顯示基質金屬蛋白酵素在急性胰臟炎合併感染時有高度的表現。在本研究同時顯示,脂多醣及腫瘤壞死因子與全血反應,可促進金屬蛋白酵素的釋放。依據以上的發現,基質金屬蛋白酵素可以做為急性胰臟炎合併感染時的早期指標。

    Acute pancreatitis causes systemic inflammatory response syndrome and potentially results in severe complications clinically. It is generally believed that cytokine storm plays a major role in the pathogenesis of acute pancreatitis. PMN-mediated multiple organ injury requires extracellular matrix degradation. MMPs are reasonable considered to involve the process of tissue invasion by PMNs.
    In this study, we demonstrated that repeated dose of intra-peritoneal cerulein injection induced acute pancreatitis in mice. Furthermore, we established a new animal model that mimic acute pancreatitis complicated with intra-abdominal infection. In addition, we investigate the expression of MMP-9 in different animal models of acute pancreatitis with or without intra-abdominal infection. The results showed that MMP-9 was significantly expressed in acute pancreatitis combined with intra-abdominal infection. The data presented in this study indicate that MMP-9 are released in whole blood in response to LPS and TNF-???| According to the above findings, MMP-9 can be a useful marker in detecting early infection during acute pancreatitis.
