    Title: 恐慌症病患接受精神科治療前後半年醫療使用率的變化
    Service Utilization of the Patients with Panic Disorder Before and After the Psychiatric Treatment

    Abstract: 本研究欲探討恐慌症病患接受精神科治療前、後半年醫療使用率的變化。本研究設計採回溯性的研究設計。研究資料取自2003年至2005年國衛院出版之全民健康保險研究資料庫之門診處方及治療明細檔(CD檔)。研究對象為2004年當中所有被診斷為恐慌症(ICD-9CM編碼為300.01)的病患,排除2003年曾被診斷為恐慌症的病患後,進一步排除小於18歲的病患,所得之15,690位恐慌症病患。所得資料經統計分析後發現,恐慌症病患的急診就醫次數及急診醫療費用,在接受精神科確診治療後半年比前半年顯著地減少。但恐慌症病患的門診就醫療次數及門診醫療費用,在接受精神科確診治療後半年則比前半年顯著地增加。另外,包含急診及門診的總就醫次數及總醫療費用,在接受精神科確診治療後半年也比前半年顯著地增加。因此,本研究結果說明精神科的治療雖然無法減少恐慌症病患的醫療使用率及醫療費用,但確可以明顯地改善恐慌症病患的急診就醫次數及費用。

    This study was aimed at investigating the utilization change of patients with panic disorder after 6 months psychiatric treatment. Retrospectvie design and recruited 15690 patients meeting ICD-9CM criteria for panic disorder from January 2004 to December 2004 with no psychiatric diagnosis for panic disorder in 2003. The data with outpatient services and emergency services utitlization of the patients with panic disorder from 2003 to 2005 were provided by the National Health Insurance database. Analyses found that emergency visits and the costs of patients with panic disorder had significantly decreased after 6-months psychiatric treatment as compared to 6 months before the psychiatric diagnosis. However, the outpatient visits and the costs of patients with panic disorder had significantly increased after 6-months psychiatric treatment as compared to 6 months before the psychiatric diagnosis. Also the total utilization of emergency services and outpatient services and the total costs had significantly increased. In conclusion, although psychiatric treatment for patients with panic disorder could not reduce the total service utilization and total costs, but effectively reduced emergency visists and the costs.
