    Abstract: 護理人員,居於整個醫病關係的樞紐地位,
    也是防杜醫療疏失的最後一道防線,如何降低藥
    事與護理給藥作業上的錯誤,對提升病人安全有
    最直接與重要的意義。本研究計畫建構在『以病
    人為中心』, 落實『提升病人用藥安全』的理念
    之下,整合醫學工程、醫療資訊、護理、藥事四
    大相關領域的技術,進行符合現行採用單一劑量
    投藥服務的『e 化醫護流程再造』:讓藥物從醫師
    開立處方簽,經過藥局調劑藥物送至護理站,經
    護士遞送到病人手中並確認其服用,乃至門診病
    患返家服藥,能在這套資訊系統的監控或協助下
    確實完成。
    本『e 化醫護流程再造工程』分為三個主軸:
    1.)e 化醫護資訊系統基礎建設,利用網路服務
    之簡易物件存取協定技術,整合醫院現有的眾多
    異質醫療資訊系統。2.)e 化單一劑量投藥服務
    系統,包含資訊化藥櫃及內建智慧型藥盒之行動
    資訊護理工作站,輔助藥事人員快速與正確的配
    藥。3.)e 化藥物資訊與稽核系統,提供護理人
    員透過條碼或無線射頻辨識技術,落實關鍵之三
    讀五對給藥作業。
    Nurses being on the pivotal positions act as a
    final defense to prevent medical errors from
    happening. In this project, we re-engineer the Unit
    Dose Distribution Services that builds on the
    concept of “patient center.” We integrate
    multidiscipline the know-how from Biomedical
    Engineering, Medical Informatics, Nursing, and
    Pharmacy fields, to design and develop an
    “e-Medication Process Re-engineering, eMPR.”
    The goal is to provide an effective and accurate
    medication process to reduce medication errors and
    improve healthcare quality.
    The entire study includes three parts: 1.)
    e-Medical Information System Infrastructure. The
    first part contains the study of heterogeneous
    medical information system and constructs the
    integration infrastructure; 2.) e-Unit Dose
    Distribution Services. It includes the use of barcode
    or RFID together with the wireless network to
    implement the three-reads, five-writes process; and
    3.) e-Medication Information and Monitoring
    Processing. The final part of this grand project will
    examine its potentials by exploring detail drug with
    each other interactions, their side effects, etc.
